Botox for Retraction

Many men experience “penile retraction,” where the penis pulls back into the body due to cold, stress, or physical activity. This is often driven by an overactive cremasteric reflex or hyper-contraction of the smooth muscle within the fascial layers of the shaft.

At BDE Clinic, we use Penile Neuromodulation—the strategic use of Botox (Botulinum Toxin)—to relax these involuntary contractions, allowing for a more consistent, relaxed flaccid length and improved “locker room” confidence. This needs to be differentiated from penile tox that many other providers offer for ED treatment and to improve erectile function which involves injecting neuromodulators directly into the corporal bodies of the penis themselves to promote vasodilation. Let me just tell you from first hand knowledge that one works and the other does not.

How It Works

Botox works by temporarily blocking the nerve signals that tell muscles to contract. When applied to the penile anatomy, it provides several key benefits:

• Reduced Retraction: Prevents the “shrinkage” reflex, keeping the penis and scrotum in a more consistently relaxed, descended state.
• Enhanced Flaccid Appearance: By relaxing the smooth muscle, the flaccid shaft often appears fuller and hangs lower. The same goes for the scrotum because the same muscles that pull up the testicles will also contribute to some penile retraction. “Like just getting out of a hot steamy shower or sauna”
• Improved Comfort: For men who experience discomfort from tight retraction during exercise or high-stress environments.

The Procedure: What to Expect

The neuromodulation procedure is quick, nearly painless, and requires no downtime.

1. Topical Numbing: A high-strength numbing cream is applied to the shaft. (If done with the BDE-Girth™ procedure, you will already be numbed during that procedure)
2. Precision Delivery: Using a small needle, we deliver small units of Botox into the superficial layers of the penis where the retractile muscles reside. We also inject the cremaster muscles in both sides of the proximal scrotum.
3. Onset: Results typically begin to appear within 5–7 days, with full relaxation achieved by day 14.
4. Longevity: Most men find that a single session provides relief from retraction for 3 to 6 months.

While the fillers provide the physical volume and girth, the Botox ensures that the fillers set while the penis is in its most elongated state.
